This doesn't work out of the box with the Unity Terrain Paint Trees feature. This is fixable, however, if you make your own prefab (don't use the ones in this package) and add the fbx model as a child of your empty prefab object. Then add an LOD Group to the parent prefab and add the tree fbx and the child leaves node to your LOD group renderers.
My midling review is just that there are 13000 tris for the tree trunk alone and 1900 tries for the leaves. I wish this model was a little more efficient since I think this model looks best when not up close, so a lower poly count would have been better for me.

The tree models are positioned horizontally and cannot be modified, making them unsuitable for painting vertical trees in a terrain.
